Sonar is easy to apply as a diagnostic method in obstetrics and gynaecology by direct contact scanning through a film of olive oil applied to the abdominal wall. Using the full bladder technique one can map out the uterus and its contents in very early pregnancy revealing the growing gestation sac from before the sixth week of pregnancy onwards. The level of nidation of the ovum and its relationship to subsequent miscarriage can be studied and the diagnosis of “blighted ovum” is facilitated at an early stage.

The diagnosis of hydatidiform mole and the estimation of retained products of conception following abortion are now commonplace. Twins can be diagnosed very early in pregnancy by the demonstration of two gestation sacs. Later on the finding of two heads on two-dimensional scanning makes the diagnosis even more certain. The growth of the foetus can be studied by plotting the enlargement of the biparietal diameter, and localisation of the placenta has become a routine and reliable procedure. Using the Doppler echo effect the foetal heart can be picked up from the 13th week of pregnancy onwards.

Approximately 30 per cent of all in-patients in the Queen Mother’s Hospital are now investigated by sonar at some stage or other in pregnancy. In gynaecology our experience continues and we have not had to alter our diagnostic criteria as applied to ovarian and uterine tumours, the different varieties of ascites, splenomegaly and hepatic infiltration with metastases.

A plea is made for sonar being accepted as an ancillary diagnostic technique by the radiological world already accustomed to visual diagnostic techniques.

  1. Do ask the patient’s permission to visit before you arrive. Ask them to be candid with you, and if they prefer you not visit, ask them if another day would be better, or if they would prefer you visit once they get home. Many patients love visitors, but some just don’t feel up to it. Do the patient the courtesy of asking permission.
  2. Do wash your hands and sanitize them before you touch the patient or hand the patient something you’ve been touching. If you wash your hands, then touch something else, like a telephone or TV remote or even the bed linens or your jacket, wash your hands and sanitize them again. Infections come from almost any source and the pathogens can survive on surfaces for days. 1 You can’t risk being responsible for making your favorite patient even sicker than they already are.
  3. Do take balloons or flowers as long as you know the patient isn’t allergic to them and is in a room by themselves. If your patient shares a hospital room, you won’t want to take either, because you don’t know if the roommate has an allergy. Most solid color balloons are latex, which is rubber, and some people are allergic to rubber. When in doubt, take mylar balloons or don’t take any at all.
  4. Do consider alternatives to balloons and flowers. A card, something a child has made for you to give to the patient, a book to read, a crossword puzzle book, even a new nightgown or pair of slippers are good choices. The idea isn’t to spend much money; instead, it’s about making the patient feel cared for without creating problems that might trigger an allergic reaction.
  5. Do turn off your cell phone, or at least turn the ringer off. Different hospitals have different rules about where and when cell phones can be used. In some cases, they may interfere with patient-care devices, so your patient can be at risk if you don’t follow the rules. In other cases, it’s simply a consideration for those who are trying to sleep and heal and don’t want to be annoyed by ringtones.
  6. Do stay for a short time. It’s the fact that you have taken the time to visit, and not the length of time you stay, that gives your patient the boost. Staying too long may tire them out. Better to visit more frequently but for no more than a half hour or so each time.
  7. Do leave the room if the doctor or provider arrives to examine or talk to the patient. The conversation or treatment they provide is private, and unless you are a proxy, parent, spouse or someone else who is an official advocate for the patient, that conversation is not your business. You can return once the provider leaves.

  1. Don’t enter the hospital if you have any symptoms that could be contagious. Neither the patient nor other hospital workers can afford to catch whatever you have. If you have symptoms like a cough, runny nose, rash or even diarrhea, don’t visit. Make a phone call or send a card instead.
  2. During flu season, it is not uncommon for hospitals to restrict visitors to spouses, significant others, family members over 18, and pastors, so it is worthwhile to call the hospital before your visit.2
  3. Don’t take young children to visit unless it’s absolutely necessary. Even then, check with the hospital before you take a child with you. Many hospitals have restrictions on when children may visit.
  4. Don’t take food to your patient unless you know the patient can tolerate it. Many patients are put on special diets while in the hospital. This is especially true for those with certain diseases or even those who have recently had anesthesia for surgery. Your goodies could cause big problems.
  5. Don’t visit if your presence will cause stress or anxiety. If there is a problem in the relationship, wait until after the patient is well enough to go home before you potentially stress them by trying to mend that relationship.
  6. Don’t expect the patient to entertain you. Your friend or loved one is there to heal and get healthy again, not to talk or keep you occupied. It may be better for your patient to sleep or just rest rather than carry on a conversation. If you ask them before you visit, gauge their tone of voice as well as the words they use. They may try to be polite, but may prefer solitude at this time instead of a visit.
  7. Don’t stay home, on the other hand, because you assume your friend or loved one prefers you not visit. You won’t know until you ask, and your friend or loved one will appreciate the fact that you are trying to help by asking the question.
  8. Don’t smoke before visiting or during a visit, even if you excuse yourself to go outdoors. The odor from smoke is nauseating to many people, and some patients have a heightened sense of smell while taking certain drugs or in the sterile hospital environment. At most, it will cause them to feel sicker, and if your friend is a smoker, you may cause them to crave a cigarette, and that might be problematic.

KENRIDGE’S STORY – Tygerberg Animal Hospital Kenridge

Established in 1992, this branch at 78 De Bron Ave has earned itself an enviable reputation in not only treating small animals, but also reptiles, fish, rodents and exotics. Patients that have become quite commonplace are snakes, tortoises, rock monitors and rabbits. Run by Dr Stephen Smith, Kenridge has special heated snake/tortoise cages, heating pads and facilities for special anaesthetic requirements.

Following the recent renovations, clients now have the use of a new and much larger parking area in Mildred Road, with easy access to the clinic via a new entrance. Kenridge now boasts a new reception and retail area with separate dog and cat waiting areas, plus separate dog, cat and exotics hospital wards. Of importance is a state-of-the-art theatre, as well as new prep room, dental/non-sterile procedure room, laboratory and pharmacy.

Complementing Dr. Smith at the practice are veterinarians Dr Liesl Shaw, Margaret Haddow and Donna Brook, as well as eight staff members.

What We Do

Our primary goal is the improvement of animal health and this takes many forms. In our consultation rooms we provide primary health care, ranging from vaccinations to deworming and flea treatment, in order to maintain the health of your pets as well as to prevent diseases.

Sick animals are checked in consultations where the client’s concerns as well as the history of the pet are discussed. Upon examining the animal, decisions are made as to whether any further diagnostic tests are needed and what treatment options are available. Some patients with less severe ailments are treated at home or on an outpatient basis. The more serious or complicated cases may be admitted for in-hospital treatment, further tests or surgery.

Many different surgeries are done on a daily basis at all our branches, but the most routine include sterilisations (spaying or castrations), stitching up wounds and the removal of growths or tumours. These are done under very sterile conditions in special theatres to prevent infections. For more specialised procedures you may be referred to one of the bigger branches.

In-hospital patients are regularly monitored and appropriate treatment is given. Further testing may be necessary for accurate diagnosis, and many of these, including X-rays, ultrasounds, blood and other laboratory tests, can be done at the bigger branches.

At our Bellville branch we have vets on the premises 24 hours a day for emergency and intensive care cases.

HISTORY – Tygerberg Animal Hospital Kenridge

A heritage as diverse and remarkable as all the patients we’ve treated with care and love over the years marks the Tygerberg Animal Hospital history.

Way back in 1954 Dr John Dorrington opened his veterinary practice in Teddington St, Bellville. Nine years later Dr Wendel du Buy joined him and the practice in Kort St, Bellville operated as Dorrington & Du Buy. In 1965 the practice had to be registered as a hospital in order to purchase drugs – which used to be bought from the local chemist – therefore the name Tygerberg Animal Hospital was born. Dr Wim Peters joined Drs Dorrington and Du Buy in 1967.

Durbanville branch saw the light of day in 1974, while Parow branch was opened in 1984. In the 1980s Drs Du Buy and Strong started the first clinical pathology clinic for animals by sending blood to pathologists in order to set up a blood test database for dogs and cats.

The Choice on Termination of Pregnancy Act No. 92 of 1996 gives all women the right to a free abortion (termination of pregnancy), at a government hospital or clinic during the first 3 months of pregnancy.

Women of all ages have the right to an abortion and should never be denied the service because of their age. It’s, however, advisable to be accompanied by a trusted adult (a parent, aunt or teacher) for support if you’re under 18.

  • Medical Abortions

Medical abortions use medication (pills or tablets) only. It’s available for women who are between 4 and 9 weeks pregnant. The length of the process is different for each woman but it usually takes a few days. Woman may experiences symptoms similar to a miscarriage.

  • Surgical Abortions

Surgical abortions involve a nurse (up to 12 weeks of pregnancy) or a doctor (from 12 to 20 weeks) conducting a same-day procedure in a clinic or hospital.

How can you access the service?

If you’re a first-time visitor to a health facility, you will be asked to complete a form and a folder will be opened.

You will need the following:

  • Your official ID document.
  • Your referral letter (if relevant).
  • Any medication you are taking.
  • Your clinic/hospital card if you have previously registered at the facility.

You must sign an informed consent form giving permission for the abortion to be done.

  8. Avoid drinking alcohol for 48 hours following your treatment, as alcohol can increase the risk of heavy bleeding.

Casualty

The casualty and trauma unit is equipped to handle all situations ‐ no emergency is too small or too big! Trauma trained doctors and staff members are available day and night ‐ all year round.

This unit has been upgraded to a Trauma Level 1 Unit ‐ competent to handle any emergencies! A triage system is in place to ensure that all patients are assessed effectively to ensure medical assistance is given in the shortest time possible. Our doctors are ATLS trained.

We have a specialist on 24 hour call to immediately attend to trauma patients.

A helicopter-landing pad ensures that patients in urgent need of medical care can be set down at Arwyp where they will immediately receive the necessary attention.

The trauma unit is fully supported by our competent team of specialists.

Workmen’s Compensation Act (WCA) cases All WCA cases are handled efficiently at the Arwyp`s 24 hour casualty and trauma unit. Special arrangements can be made with the unit manager to ensure treatment to WCA patients without delay.

Day Surgery

For some procedures it is not necessary that the patient remain in hospital overnight. These patients are therefore admitted and discharged on the same day. The day ward is located on the 12th Floor.

Information for day patients Please prepare yourself for a waiting period and bring reading matter or something to keep yourself occupied. After your operation you will recuperate in the ward. The minimum period of stay will be one and a half hours and can be extended depending on your reaction to anaesthetic and the type of procedure performed.

Cardiovascular surgery

The purpose of coronary artery bypass surgery is to improve the blood supply to an area of the heart that has been deprived of adequate circulation. Two procedures are involved: an incision is made in the leg to remove a vein which is then used as a graft to create a new coronary artery. the second incision is made in the chest to enable the surgeon to reach the heart. More than one artery can be blocked and it is then necessary to insert more than one graft.

Terms like double bypass, triple bypass and quadruple bypass indicate the number of grafts required. Coronary artery bypass surgery takes anything from three to six hours, depending on the number of bypasses being inserted. The above procedures are all performed in a cardiac catheterization laboratory also known as a cathlab.

Gastroenterology

It involves a detailed understanding of the normal action of the gastrointestinal organs including the movement of material through the stomach and intestine, the digestion and absorption of nutrients into the body, removal of waste from the system and the function of the liver as a digestive organ. It includes common and important conditions such as colon polyps and cancer, hepatitis, gastro-oesophageal reflux (heartburn), peptic ulcer disease, colitis, gallbladder and biliary tract disease, nutritional problems, Irritable Bowel Syndrome (IBS), and pancreatitis.

General Surgery

Once a patient has been admitted to hospital for general surgery, the doctor and the nursing staff will give the patient more specific instructions.

The doctor may do some blood tests, an ECG or take chest x-rays. A full medical and anaesthetic history will be taken and any risk factors reported to the doctor.

A patient may not eat or drink for eight hours prior to surgery. A consent form is signed and a hospital theatre gown is to be worn to theatre.

Any jewellery, dentures, contact lenses and nail polish must be removed.

The anaesthetist may order pre-medication and a urine specimen will be sent to the laboratory for testing.

The patient may need to be shaved or receive a specific skin preparation prior to the procedure and identification bands will be fitted.

Hepato-pancreatobilieary surgery (ERCP’s)

ERCP is a procedure that enables your physician to examine the pancreatic and bile ducts. A bendable, lighted tube (endoscope) about the thickness of your index finger is placed through your mouth and into your stomach and first part of the small intestine (duodenum). In the duodenum a small opening is identified (ampulla) and a small plastic tube (cannula) is passed through the endoscope and into this opening. Dye (contrast material) is injected and X-rays are taken to study the ducts of the pancreas and liver.

What Preparation is Required?

Your stomach must be empty, so you should not eat or drink anything for approximately 8 hours before the examination. Your physician will be more specific about the time to begin fasting depending on the time of day that your test is scheduled.

Visitors to the hospital are a welcome and important part of the patient’s recuperation. However, to ensure that the patient receives adequate rest and for the smooth running of the hospital, certain restrictions may be necessary. For the comfort and safety of all patients please adhere to the following rules:

Visitors must enter through the hospital’s Main Entrance or the Pavilion entrance on Maple Street. Once you arrive, you’ll be issued a visitor badge specific to the patient or area you are visiting. Visiting hours are from 9 a.m. to 9 p.m. Visitors needing to enter the Medical Center between the hours of 9 p.m. and 5 a.m. must enter and check in through the Emergency Department waiting room entrance and obtain a Visitor Pass. Permission to enter during non-visiting hours is only granted under specific circumstances and must be obtained in advance by the patient’s nurse.

We recognize the important role family and loved ones play in the healing process, and we encourage visitors to come when it’s best for their loved one.  When visiting we ask: 

  • That you respect patients’ preference regarding visitation 
  • Limit visits during time of personal care and sensitive discussions

The hospital is open 24 hours a day, seven days a week.  For the safety of patients and employees, public entry, exterior doors (except the north lobby) at St. Joseph are locked from 8:30 p.m. to 6:00 a.m. During this time, all visitors must enter and exit the building through the north lobby emergency department entrance and check in with security to receive a visitor badge.

St. Joseph General Visitor Guidelines

  • To protect our patients, we ask that if you have a fever, a cold, sore throat, or flu, please do not visit.
  • Wash your hands or use hand sanitizer before and after entering a patient room.
  • The number of visitors at a patient’s bedside is limited to two at any time.
  • Children under the age of 16 are not allowed to visit patient care areas. (Children may wait with an adult in the first floor lobbies of the main hospital and patient pavilion, or in the cafes.)
  • Patients may be asleep at any time, so please be considerate and keep noise to a minimum.
  • Please respect the privacy of other patients and their guests while visiting.
  • Visitors may be requested to leave a room at the discretion of the nursing or medical staff.
  • Cell phones are allowed, however their usage may be restricted if noise, photo taking or other use impacts patient care or privacy.
  • Smoking and tobacco use is not permitted anywhere on the St. Joseph’s campus, including buildings, grounds, parking areas, and walkways.

Area Specific Visitor Guidelines

The following areas of the hospital have specific visitor guidelines due to the nature of their care:

  • Maternity – Brothers and sisters of the newborn are welcome to visit during Sibling Visiting Hours from 5 to 8 p.m., accompanied by an adult. Short visits are best for children. The number of visitors at the bedside in Maternity is limited to three people at any given time. Visitors must not have fever, cold, cold sores, diarrhea, vomiting, rash or skin sores. Important – Visitors who have been exposed to illnesses such as chickenpox or measles may not visit.
  • Neonatal Intensive Care – Parents are encouraged to visit often and designated grandparents may visit as well. Parents and visitors may be asked to leave during procedures, shift report, and physicians’ rounds.
  • Pediatrics – Parents or guardians may visit at any time. Shorter visits with friends and extended family are best for the pediatric patient. Visitors must be at least 16 years of age. One parent may stay in the room with the patient at night.
  • Intensive Care (ICU) – Visitors are permitted according to the patient’s condition and will be determined by the physician or nursing staff. The length and time of visits may be adjusted by the nurse caring for the patient.

Cardiac Catheterisation Laboratories (Cath Labs)

Our two Cardiac Catheterisation Laboratories provides all the features of a modern catheterization laboratory. We offer diagnostic and interventional cardiology, including but not limited to coronary angioplasty, coronary stenting, advanced pacemaker implantation technology, as well as specialised paediatric cardiology and electrophysiology.
